A Young Person’s Thoughts on Transition

A young adult with CF who recently transitioned from PCH to SCGH, has kindly shared their experiences and advice to help other teens feel more informed and confident as they prepare for this next step in their CF journey.

“Moving from PCH to Sir Charles Gairdner can feel daunting especially when you’ve been with the same team for years, but transitioning doesn’t mean you’re losing the care and support you’ve had. It means moving into the next stage of your adulthood with a team that is there to support you as an adult.

You don’t have to know everything straight away. It’s okay to ask questions. Take your time getting comfortable with the new team. They are here to support you through the change.

I was worried about having a new team after being with the same team for so long, but everyone at Charlie’s are kind-hearted and want to support and lookout for what is best for you.

I was also worried about having to be more independent, but in the end I prefer it, and the team are very accommodating for your needs, working around your work hours and activities.

Some things I wish I knew when I was transitioning, is that both hospitals talk to each so they have your file and know a lot about you already, so it’s ok if you don’t remember all the types of medicine you take.

There is nothing to be stressed about transitioning, and in the end, I appreciate and enjoy the team at Charlie’s. They only want what is best for you.”
