New Perth Children’s Hospital CF Pharmacy Pre-Order Service

Dear CF Families
We are very pleased to announce that we are ready to go with our CF Pharmacy Pre-Order Service!
The CF pharmacy pre-order service is a system for pre-ordering prescriptions from our doctors and medications from pharmacy. We hope that the pre-ordering system will reduce pharmacy wait times for families affected by CF by reducing the workload on the PCH dispensary on clinic days. It is important to know that not all medications are included in the drop down list as the service is intended for routine, ongoing medications that rarely change. Periodic and specific medications will need to be requested in clinic as per the current process.

Each medication will need to be selected individually from each drop down box. Once you have completed all sections of the form including your child’s name, date of birth, current weight and expected collection date, submit the form online or save the document and email the completed form to pch.respmedpreorder@health.wa.gov.au
Please ensure the form is submitted before 11am on the Monday before your next clinic appointment, or if you require medications in-between clinic appointments so that the script can be written at our weekly meeting. Your script will then be sent to the pharmacy on your behalf, to be dispensed and ready for you to collect on your specified date. If you are unable to collect your ordered medication please call the PCH pharmacy on 6456 0190 to advise of an alternative collection time and date.
We have also recently developed a PCH CF website pch.health.wa.gov.au/Our-services/Respiratory-and-Sleep/Cystic-Fibrosis. This website will continue to be updated with useful information which includes a link to the pharmacy pre-order form.
