Hospital Transition Workshop
We are hosting an online Hospital Transition Workshop to support teens with CF and their families as they prepare for the move from Perth Children’s Hospital (PCH) to Sir Charles Gairdner Hospital (SCGH).
The workshop will explain what to expect during the transition process, how CFWA can support you, answer common questions and provide information about the supports available to patients at SCGH.
Members of both the PCH and SCGH CF teams will also be joining the session, providing a great opportunity for teens and families to hear directly from the teams involved in their care.
